Welcome to the final day of the Health Dimensions Group National Summit! This morning, the attendees awoke bright and early to begin the final day with a power walk and a breakfast with sponsors.
The morning then kicked off with a Plenary Session discussing technology in the care continuum. Doug Busch, Senior Vice President and Chief Operating Officer of Intel-GE Care Innovations and David Classen, Associate Professor of Medicine at the University of Utah led the discussion. The session included information about technology integration that can work to improve quality and reduces costs in acute and post-acute environments, as well as technology’s major impact in the coming years on the health care delivery system and the ability of patients to self-manage their conditions.
Following a refreshment break with sponsors, Ken Coburn, CEO and Medical Director of Health Quality Partners, addressed the guests in a Plenary Session about Care Pathways Across the Continuum, which addressed what is required to ease patient transitions from one service or site of care to the next, among other related topics.
The afternoon was then marked with a Panel Session discussing the Post Acute Continuum with an ACO or Integrated Delivery System. The panelists included Janet Tomcavage, Vice President of Health Services at Geisinger Health Plan; Robert Palmer, Director of the Glennan Center for Geriatrics and Gerontology at the Eastern Virginia Medical School; Mark Enger, Vice President and COO of Care Delivery at Kaiser Permanente Northwest; and Audrey Weiner, President and CEO of Jewish Home Lifecare.
