Organization will present best practice case study at LINK Conference in July

Manitowoc Health and Rehabilitation Center, a skilled nursing facility (SNF) managed by Health Dimensions Group, is proud to announce that it is has earned the LTC LINK Spirit of Innovation Award for the 7th Annual LTC LINK Conference. This award recognizes companies for their innovation, dedication to continuous improvement, development of best practices, and commitment to the success of the long-term care industry.

“It is certainly an honor to be recognized by our industry peers,” said Julie Place, administrator at Manitowoc Health and Rehabilitation Center. “This award is reflective of our staff’s professionalism and most importantly, their commitment to our patients.”

Working with local hospitals, Manitowoc Health and Rehabilitation Center developed clinical pathways for chronic diseases, organizing care from the start of the patient stay and extending 30 days post SNF discharge. Combining extensive staff training with physician collaboration and comprehensive education for the patient and family, Manitowoc decreased readmissions, reduced overall medical costs and demonstrated success with quality measures like functional status improvement, increased rates of successful discharge and customer satisfaction. A representative from the Center will present a case study at the 2011 LTC LINK event this July 25-27 at the Swissotel Chicago.

“The goal of the Spirit of Innovation Award is to help advance business productivity and foster innovation in long-term care. These winners have not only developed unusually successful business initiatives but have also demonstrated a willingness to share their great ideas with others in support of this mission,” said Scott Brown, Vice President and Conference Director for LTC LINK.

Winners were selected based on a number of factors, including level of innovation, feasibility and business impact.
