Building an All Payer Geriatric Care Coordination Program: The Experience of One PACE Program, will be presented by Jade Gong and Richard DiTommaso, MHA, Community LIFE, on Monday October 27th from 10-11:30.
Creating a Framework for Excellence: A Look at Decreasing Variation and Increasing Uniformity In and Across PACE Practice, will be presented by Anne Lewis and Kelly Hopkins, BSBA, CHE Trinity Health on Tuesday, October 28th from 3:30-5:00PM.
