As health care evolves from hospital-centric to patient-centric, the care continuum must also change from venue-based to seamless. The eighth plenary meeting featured a panel discussion of thought leaders in post acute care.  Each presenter discussed their strategies to create value-based relationships with hospitals, physicians, and payers and a care continuum that will meet the needs of elders while concurrently preparing for a future of accountable care.

Presentation Summary: Diane Peters and Jane Gorwin on Hospital Readmissions

Diane Peters, Senior Consultant, Health Dimensions Group and Jane Gorwin, Senior Health and Hospice Consultant, Health Dimensions Group, followed Edeburn to discuss measuring and managing hospital readmission from post acute venues.

They reviewed how leading providers are measuring their readmission rates and what operational strategies, procedures, tools, education and protocols, they are finding effective in reducing readmissions as well as what hospitals and health systems look for and expect when evaluating post acute providers as potential partners in their care continuum.
